Insurance Coverage
We do not bill insurance. A super bill will be given to you at the end of
your visit. You can submit that to your insurance company for direct
reimbursement. It is your responsibility to inquire with your insurance for
benefits and coverage.
We are not Medicare providers.
Cancellation Policy
Please cancel at least 24 hours in advance if you have a conflict.
If you cancel with less than 24-hour notice, you will be charged for half of
the office visit, unless we are able to fill your spot.
